This registration will allow you to provide both player and parental contact information and sign consents. Creating an account and filling in this form will allow you access to your child's schedule and other team-specific information.
You must fill in this form prior to proceeding to the Ontario Lacrosse Association registration portal. A link to the portal will be provided at the end of this registration page.
Important: All players who have not previously used the OLA Registration Portal must upload of a birth certificate, passport or health card to verify age into the OLA Registration Portal.
Payment will be made online by credit card or if you wish to pay cash or cheque this can be done at one of our New Player Information Sessions. If paying cash or cheque, please complete the registration process up to the payment screen and then log out. It will make your in-person session very easy and quick. The OLA Portal replaces the need for each participant to sign a 'card' in order to participate. You must visit both this registration and the OLA Portal! Your child will not be registered for the 2019 season until you do both.
Offline Payment Methods: Visa and MasterCard will be accepted along with cash and cheque at the New Player Information Sessions.
